We ask that if you wish to make an appointment for an in-person session that you evaluate your exposure and health honestly and only schedule a session if you:

• Have you not been experiencing any cold/flu-like symptoms within the last 14 days? (Fever, fatigue, coughing, sneezing, stomach pain, diarrhea, loss of taste or smell, shortness of breath, chest heaviness or pain, unusual rash, sore muscles, etc.)

• Have not been in close proximity or direct contact with anyone exhibiting cold/flu-like symptoms or that has been diagnosed with COVID-19 within the last 14 days?

• Have not used public transportation within the last 14 days (air, bus, or train travel)?

• Do not consider yourself to be "at-risk" for contracting the virus? If so, please state why.
